Note: Make a new topic in the Guild request sub forum titled with your guild

Do you have over 15 members in the Guild (Y/N)?
Do you have over 8 active members in the Guild (Y/N)?
Forum:
Do your Guild members use the forum often (Y/N)?
Do you plan on using the forum often (Y/N)?
Do you understand a forum not used for 2 weeks maybe deleted (Y/N)?
Do you wish for Moderation powers in the forum (Y/N)?

Do you and your Guild members use ventrilo often (Y/N)?
Do you plan to use Ventrilo more with Guild members (Y/N)?
Do you understand a Ventrilo chatroom maybe deleted if determained that is it's active enough or been unused for 2 weeks (Y/N)?
